Matt Foltz, CEO, PT

Matt is a native of Roswell, GA, and has been practicing physical therapy for over 25 years in the Atlanta/North Metro area, specializing in orthopedics and sports medicine.

Matt received his undergraduate degree from the University of Georgia in Exercise Medicine and received his Masters of Physical Therapy degree from Emory University in Atlanta, GA. He currently assists in the interview process of prospective Physical Therapy students at Emory University.

During Matt’s 25 years in the Physical Therapy field, he has assisted in numerous sports medicine venues including the SEC football championship, SEC basketball championship, ACC basketball championship, World Wrestling Entertainment, SEC gymnastics championship, Peach Bowl, Sarazen golf tournament, and area high school sports programs. Matt previously served as Vice President of Operations for one of the country’s largest outpatient rehabilitation corporations, while still maintaining active patient care and his sports medicine outreach programs.

When Matt is not out serving in the community he is very active with his family.

Cindy is a native of Canton, CT, and has been practicing physical therapy in Cumming since 1984. She specializes in orthopedic and sports injuries.

Cindy received her degree from the Medical College of Georgia in Augusta, GA in 1984. She co-owned Lanier Physical Therapy for 10 years in Cumming, Sugar Hill, and Duluth before selling it to a large rehabilitation corporation. She has also both owned and worked for in-home physical therapy companies. Through her interest in sports, she has volunteered at both Walton and Forsyth Central High School athletic events. Her physical therapy treatment philosophy consists of an individualized treatment approach emphasizing proper exercise and posture. She also works part-time for her husband Jim, in accounting, and is involved in CASA of Forsyth County and Cumming First United Methodist Church. In her spare time, she enjoys golf, Georgia football, arts and crafts, traveling, family.

Jill is a native of Cumming, GA, and has been practicing Physical Therapy for over 20 years in the Metro area. She has been specializing in orthopedics for the past 15 years.

Jill received her Bachelor of Physical Therapy degree from Georgia State University, in Atlanta. Jill loves to help the people in her community stay healthy and active and takes pride in the small-town feel of her one-on-one physical therapy approach. As well as treating general orthopedic patients, Jill has a love for the geriatric population and performs vestibular rehab - a form of therapy that involves treating balance deficits and dizziness issues.

As a part-time physical therapist and a full-time mom of three, Jill knows the importance of getting her patients back to doing what they love and she takes much pride in helping people "rehab like a champion".

Ellen is a native of Cumming, GA, and began practicing physical therapy 14 years ago. She has worked in orthopedics, home health, and rehab facilities around the metro Atlanta area.

Ellen received her undergraduate degree in Exercise Science from Furman University and then went on to receive a Master's degree in Physical Therapy from the Medical College of Georgia.

Ellen is a busy mom with 3 children and is a competitive tri-athlete during the season.

Crystal Holcomb, PTA

Crystal has been practicing as a Physical Therapist Assistant since 2007. She is a graduate of Athens Technical College and has been licensed in the state of Georgia since graduation. Crystal has worked in a variety of settings and has a background in outpatient orthopedic care, aquatic therapy, vestibular rehabilitation, and geriatrics.

Crystal is a certified Diabetes Paraprofessional from the Association and Diabetes Care & Education Specialists and is leading our diabetes self-care management program at Champion.

Crystal is a Forsyth County native. When not working at Champion, she enjoys spending her time with her husband, William, their two sons, and golden doodle.

Jose DeJesus, PT, DPT

José is originally from North Carolina and has been practicing Physical Therapy for over 13 years. José has lived in the suburbs of North Atlanta with his wife and three kids for 10 years.

José attended East Carolina University where he received his undergraduate degree in Exercise Physiology and his Doctoral degree in Physical Therapy. Since graduation, José's practice has been centered around the treatment of sports and orthopedic injuries in the outpatient clinical setting. His special interests include general orthopedics, treatment of the shoulder and knee in young athletes, dry needling, and vestibular therapy.

José is a certified clinical instructor for future Physical Therapists through the American Physical Therapy Association. In his spare time, José enjoys spending time with his family, hiking, traveling the world, and cooking/baking.

Kayla Williams PT, DPT

Kayla was born and raised in Cumming, GA. She received her Bachelor in Kinesiology with an emphasis in Exercise Science from the University of North Georgia in 2014. She had the pleasure of staying in Dahlonega, earning her Doctorate in Physical Therapy from UNG in 2021.

Due to her involvement in sports growing up, Kayla is interested in sports rehabilitation, specifically overhead sports and post-operative care. After recurring injuries and physical therapy rehabilitation while playing sports, she hopes to share her love for rehabilitation with all ages and injury types.

Kayla lives with her husband in Dahlonega, GA. They hope to eventually move back to the Cumming area to be closer to family and where her and her husband were raised. In her spare time, she loves to travel the world, cheer on the Atlanta Braves, and spend time with her family.

Julie Epp, COOJulie Epp, COO

Julie is a native of Cumming, Ga. She attended school at Gainesville College and thereafter began her career specializing in all facets of office and insurance management related to the physical therapy field.  With 25+ years of experience, she controls the day to day operations at both Champion Physical Therapy locations.

Julie has one son and enjoys spending most of her time with family and friends. She loves traveling, attending concerts, and spending time on the lake with her fiancee.

Originally from New Hampshire, Donna has lived in Cumming for 10+ years with her husband Paul. She has two daughters and is enjoying her first granddaughter!

In her free time, she enjoys spending time with her family and traveling the world.

Dani Kwiatkowski, Front Office Manager

Originally from Maryland, Dani has lived in Cumming for 20+ years with her husband, two daughters, and granddaughter. Dani has spent most of her career in customer service and enjoys meeting and helping those who walk through the Champion doors.

When she’s not at Champion, you can usually find her on the tennis court or by the lake.

Keith Borger received his Bachelor of Science in Biology from the University of West Georgia in 2013, where he played Division II baseball for the Wolves. In 2016, Keith received his Doctorate in Physical Therapy from Mercer University. Upon achievement of his Doctorate, he completed Mercer University’s Orthopaedic Physical Therapy Residency program in 2017 and went on to achieve a Board Certification as an Orthopaedic Clinical Specialist (OCS) in 2018. Keith is currently the Clinic Director at the Alpharetta location and the Center Coordinator for Clinical Education at Champion Physical Therapy.

Keith's clinical interests and specialties include sports rehabilitation with an emphasis on overhead and swing athletes, manual therapy, movement analysis, spinal mobilization/manipulation, and post-operative care.  He is certified as both a Titleist Performance Institute (TPI) professional and OnBaseU professional, which allow him to focus on reducing injuries and improving golf and baseball performance. He understands that a properly functioning body allows a player to swing a golf club, swing a bat, or pitch in the most efficient way possible. Using these certifications, Keith can analyze how physical limitations in a player’s body can adversely affect a person's performance and potentially lead to injury.

Keith lives in Roswell, GA with his wife, two daughters, and dog Pluto. In his spare time, Keith enjoys playing golf and tennis and cheering on the Atlanta Braves, Atlanta Falcons, and Georgia Bulldogs.

Drew Parten, PT, DPT

Drew was born and raised in Royston, GA. He received his Bachelors of Science in Kinesiology from the University of North Georgia and received his Doctorate of Physical Therapy from Mercer University in 2021. Following Physical Therapy school, he is completed Mercer University’s Orthopaedic Physical Therapy Residency program.

Drew’s clinical interest include sports rehabilitation and injury prevention, joint mobilizations, and manual therapy. As a physical therapist, Drew enjoys working across the lifespan to empower individuals to achieve their goals and enhance their physical performance. He is a certified Titleist Performance Institute (TPI) professional, which allows him to focus on reducing injuries and improving golf performance.

Outside of clinic, Drew enjoys participating in various sports and outdoor activities with family and friends.

Eric Holshouser, PT, DPT

Eric is originally from Jacksonville, FL before moving to Atlanta, GA in 2017 to pursue his Doctorate in Physical Therapy at Emory University where he graduated from the Doctor of Physical Therapy and Master of Business Administration dual degree program in 2021. Prior to pursuing a career in physical therapy, Eric received a Bachelor of Science degree in civil engineering from the University of Florida and was employed as a civil engineer designing industrial land development projects.

His clinical interests include sports, orthopedics, functional movement, and applying natural movement to rehabilitation.  Eric is passionate about human performance, well-being, and longevity with much of his professional focus on giving individuals the tools to move and live well through all stages of life. He currently serves as a research advisor and adjunct professor in Emory University’s Division of Physical Therapy where his research focuses on student well-being and healthy habit formation.

Outside of the clinic, Eric is a science enthusiast and in his free time enjoys basketball, outdoor activities, and practicing Capoeira.

Kaylie Anstis, Front Office Manager

Kaylie is a Forsyth County native, living in the Alpharetta/Cumming area. She graduated from Samford University with a Bachelor of Science Degree in Health Sciences with a concentration in Healthcare Administration. Kaylie is a proud dog mom to her German shepherd, Myla.

Kaylie is an avid runner,  completing 8 Peachtree Road Races and 5 half-marathons. Kaylie loves to spend her free time on Lake Lanier, watching NFL football (specifically the Pittsburgh Steelers), reading, and walking her dog!
